The following eligibility survey will help us to connect you with the vehicle assistance programs you are most likely to be qualify for. Before you start the survey, please make sure that you meet the following prerequisites:

• You are the registered vehicle owner
• Vehicle does not have a “tampered” emissions-control system
• All appropriate registration fees for the vehicle have or will be paid to the DMV

(aborted, manual mode, and training mode tests do not qualify)
